Advanced Practice Clinician- Nurse Practitioner/Physician Assistant

HealthCare Support is actively seeking an Advanced Practice Clinician- Nurse Practitioner or Physician Assistant to fill an opening in Greensboro, NC.

Daily Responsibilities for NP/PA:
  • Travel to 4 facilities in Guilford County to perform comprehensive patient assessments, addressing both medical and behavioral conditions.
  • Manage acute and chronic illnesses proficiently, ensuring accurate and thorough documentation, including ICD 10 conditions.
  • Communicate and collaborate with the interdisciplinary care team, conducting advanced illness and care planning conversations, while providing counseling and education to patients and caregivers.
  • M-F 8am-5pm

Required Qualifications for NP/PA:
  • Active and unrestricted license in North Carolina
  • Graduate of an accredited MSN, DNP, or MPA program
  • Board certified NP (through ANCC or AANP) or PA
  • Current, active DEA licensure/prescriptive authority
  • Full COVID-19 vaccination

Preferred Qualifications for NP/PA:
  • 1+ year of clinical experience in practice (long-term care setting preferred)
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $75.00- $80.00 / hour
