Job Summary:

In this role, you'll empower clients to build strong foundations for wellness and independence, utilizing evidence-based techniques to create personalized plans that address their specific needs related to addiction and mental health. You'll connect clients with local resources to foster growth, stability, and resilience, helping them navigate the recovery process.
About the Role:
As a Community Support Specialist, you'll provide community-based services to adult clients struggling with substance use disorders, including assessment, diagnosis, treatment planning, and intervention implementation. Your work will involve collaborating with clients and other stakeholders to develop and implement individualized treatment plans that promote empowerment, skill-building, and relapse prevention.

- Provide community-based services to support clients in their recovery from substance use disorders, including assessment, diagnosis, treatment planning, and intervention implementation
- Collaborate with clients, families, and stakeholders to develop and implement individualized treatment plans that address addiction and co-occurring mental health needs
- Utilize a strengths-based approach that emphasizes empowerment, resilience, and harm reduction, incorporating evidence-based practices such as motivational interviewing and cognitive-behavioral therapy
- Coordinate care with other service providers, including primary care physicians, community organizations, and peer support groups
- Participate in staff development activities to enhance knowledge and skills related to substance use disorder treatment and adult mental health services
- Work collaboratively with team members to ensure effective coordination of care and support for clients
- Develop and maintain relationships with key stakeholders involved in the client's care, including family members, healthcare providers, and community organizations
- Prepare for and facilitate group sessions focused on recovery skills, relapse prevention, and wellness as pertinent to client treatment plan goals.
- Maintain accurate and timely records of client interactions and progress
Education, Experience, and/or Credential Qualifications:
There are several ways in which you may qualify for this role....
- Bachelor's degree in Psychology, Social Work, or other human services specialized field;
OR... - Bachelor's degree in an unrelated field with two years of related work experience;
OR... - Any four-year combination of higher education and two years of related work experience;
OR... - Associate's of Applied Science in Behavioral Health Support from and approved academic institution;
OR... - Four years of related qualifying experience in delivering services to individuals with mental health disorders, substance use disorders, or developmental disabilities.

- Current driver's license, acceptable driving record, and current auto insurance.
- Reliable means of transportation to transport clients in a personal vehicle.
- If operating a company-owned vehicle, must be at least twenty-one (21) years of age.
- If operating a company-owned 15 person passenger van, must be at least twenty-three (23) years of age.
