Salary and Benefits
- Base Salary: $105,000 – $135,000 (commensurate with experience and region)
- Incentive Bonus: $15,000 – $25,000 annually based on quality outcomes
- Physician Partnership Track available
- 6 Weeks of Paid Time Off (inclusive of vacation, holidays, and CME)
- Annual CME Allowance: $5,000

Responsibilities
- Full-Time Schedule: Monday to Friday, 8:00 AM – 5:00 PM
- Patient Load: 13–16 patients per day
- Appointment Duration:
- Routine Visit: 20 minutes
- New/Complex Visit: 40 minutes
- Preventive, Value-Based Care Model
- Support Team Includes:
- Medical Assistant
- Scribe
- Social Worker
- Onsite Laboratory and Phlebotomy
- Specialist Access via RubiconMD
- EMRs: Canopy and Greenway
- Participation in Community Health Events and Wellness Programs
- Patient Transportation Within a 5-Mile Radius
- Collaborative Setting with 3–5 Providers Per Clinic
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or equivalent accredited NP program
- Active Illinois NP License and DEA License
- National Board Certification in:
- Family Nurse Practitioner (FNP)
- Adult-Gerontology Primary Care NP (AGPCNP)
- Adult NP (ANP)
- Gerontological NP (GNP)
- Bilingual Spanish Required
- Experience with HCC Documentation, ICD-10 Coding, and Health Risk Assessments (Preferred)
