RN Home Care Case Manager Needed. BrightStar Care is Growing in the South Minneapolis Area and looking for an RN Case Manager 3-Days Per Week. $3000 Sign-On Bonus. Our Case Managers are Salaried, work with a team of nurses (both RNs and LPNs) to complete assessments, re-assessments, and some skilled cares for home care private pay and veteran clients. BrightStar Care is a non-medicare agency, so no OASIS required. Our nursing team is just that, a team and a family of nurses all looking to help keep our clients safe, happy, and healthy in their homes for as long as possible. PTO Accrual from Day 1 and Paid Mileage between clients and office. We expect this RN to have clients primarily in the Minneapolis Metro Area (Bloomington, Edina, Eden Prairie, etc). Job Responsibilities:
Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
Collaborate with multidisciplinary team members to optimize patient care and outcomes.
Educate patients and their families on health maintenance and disease prevention.
Ensure accurate documentation and compliance with all regulatory guidelines.
Support our caregiving team by providing education, answers, and supervision in the field.
On-Call Rotation amongst all nurses, primarily only up to 5 weeks/weekends per Year.
Requirements:
Active RN license in the state of Minnesota.
At least 2 years of experience working as a nurse.
Previous experience in a home health or geriatric setting is advantageous.
Exceptional interpersonal and communication skills.
Committed to delivering high-quality, patient-centered care in our clients' homes.
Must have a reliable vehicle, valid driver's license, and auto insurance.
