Job Summary:
- The Pharmacist is responsible for appropriate, evidence based, medication use for the institution. They ensure patients get the right medication at the right dose. Pharmacists must have a medication safety viewpoint and be in compliance with all applicable standards and regulations (TJC, BOP, CMS, etc). Pharmacists oversee the activities of pharmacy technicians. Pharmacists must engage in teaching as part of the hospital's mission.
Core Responsibilities and Essential Functions:
Provides clinical pharmaceutical care to patients. Prevents, detects, and resolves medication-related problems and makes appropriate evidence-based, patient-centered medication therapy recommendations. Participates as an effective member of interdisciplinary care team(s) and communicates follow-up with team. Effectively verifies patient medication orders for appropriateness and accuracy. Provides effective medication education to patients, families, team. Enhances the quality of delivery of care. Actively participates in the development and implementation of performance/quality improvement and patient safety initiatives, including medication event/adverse reaction reporting follow-up, clinical guideline development, disease management, medication use evaluation/improvement, cost containment actions. Represents pharmacy services on institutional/practice area committees, quality unit councils. Conducts monthly unit inspections of assigned area. Demonstrates proficiency in pharmacy distributive services. Demonstrates quality, safety and efficiency in the distribution process. Actively collaborates and demonstrate teamwork with pharmacy coworkers to ensure communication, excellent customer service, safe and effective drug distribution. Provides oversight for pharmacy technicians. Participates in opportunities to improve the quality, effectiveness, and/or efficiency in distributing medications. Supports the education and professional development of the Pharmacy Department and other hospital disciplines (nursing/medical/etc.). Precepts residents, pharmacy students, or pharmacy technicians as assigned. Participates colloquially in the development of new programs, services, and practices, educational activities of the pharmacy department and medical team. Engages in professional self-development. Completes medical center/pharmacy competency requirements by deadlines. Establishes regular meetings with manager. Enhances professional and technical knowledge/competency by attending seminars, continuing education programs, department meetings, etc. Demonstrate active membership/service/leadership in professional organizations
Required Minimum Education:
- or Bachelors Pharmacy
- Doctorate Pharmacy-Preferred
All certifications are required upon hire unless otherwise stated.- Reg Pharmacist within 180 Days
Required Minimum Experience:
Minimum 2 years in Pharmacy as a pharmacy technician, intern and/or pharmacist Preferred
Required Minimum Skills: - Provide independent, direct patient care with an expanded scope of practice using a collaborative drug therapy management (CDTM) protocol. This enables the clinical pharmacy specialist to initiate, adjust, and discontinue medications, order and monitor laboratory studies, and perform limited physical assessment. Current referral criteria are heart failure, hyperlipidemia, hypertension, primary/secondary prevention and cardiometabolic risk reduction, medication management/polypharmacy, and drug information requests.
In addition to direct patient care activities, this position is responsible for ensuring appropriate medication therapy and clinical outcomes for the institution related to specialty area. Monitors drug therapy regimens and works closely with clinic staff to assess, monitor and recommend appropriate drug therapy as applicable. There are many opportunities to precept students, PGY1 residents, and a PGY2 Ambulatory Care resident, as well as opportunities for involvement in research and protocol development.
Preferred applicants will have completed a PGY1 OR a PGY2 Cardiology OR a PGY2 Ambulatory Care program.
