Job Summary:

A leading healthcare organization is seeking a highly experienced Director of Case Management to lead and manage all functions within the Case Management Department. This key leadership role ensures seamless care coordination, efficient discharge planning, regulatory compliance, and optimal resource utilization across the care continuum.

This position requires a strategic thinker and dynamic leader with a solid RN background, executive-level experience, and a proven record of success in complex healthcare environments.

Key Responsibilities:
  • Oversee daily department operations, including staffing models, technology, onboarding, and team competencies.
  • Ensure care coordination and discharge planning processes are timely, well-documented, and high-quality.
  • Partner with financial and clinical leadership to align case management operations with organizational goals.
  • Monitor performance data, identify areas for improvement, and implement sustainable process enhancements.
  • Maintain relationships with internal and external stakeholders including compliance, legal, and executive leadership.
  • Ensure adherence to internal policies and external regulatory standards.
  • Foster a culture of excellence aligned with the organization's values and code of conduct.
  • Additional duties as assigned.
Qualifications:
  • Licensure: Must be a Registered Nurse (RN); LCSW certification is not accepted.
  • Education: Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
  • Experience: Minimum of 2 years of recent Director-level experience in Case Management within a large acute care hospital.

  • Proven success in executive-level collaboration and results-driven leadership.

Compensation & Benefits:
  • Salary Range: $110,000 - $130,000 (heavily based on years of experience)
  • Sign-On Bonus: $10,000
  • Relocation Assistance: Case-by-case basis
  • Bonus: 15% target bonus (based on performance)
