Physical Therapist - Home Health

Location

Groverton, NH to Pittsburg, NH

Assignment Duration

This is a 13-week contract position.

Schedule

8:00 AM - 5:00 PM, Monday - Friday; around 8/9 AM start.

Job Description

A Physical Therapist is needed for a Home Health Hospice Agency covering northern territories. Candidates must possess a valid vehicle and be comfortable driving in winter weather conditions. Availability for travel is essential, and applicants must reside more than 50 miles from the facility.

Responsibilities

  • Provide physical therapy services to patients in their homes.
  • Evaluate patient needs and develop individualized treatment plans.
  • Document patient progress and adjust care plans as necessary.
Required Experience / Certifications / Licensure
  • 2+ years of current experience in Physical Therapy in Home Health.
  • Experience with age groups ranging from Pediatrics to Geriatrics.
  • Previous travel experience highly preferred.
  • Home Health and/or Hospice experience required.
  • OASIS experience required.
  • Active American Heart Association BLS certification required.
  • Valid New Hampshire state license required.
