Position Overview: We are seeking a dedicated Counselor (LGSW or LGPC) to join our team in Southeast DC. The ideal candidate will provide assessments, counseling, and case management services as part of a multidisciplinary care team. This is a hybrid role with both in-person and remote responsibilities.
Responsibilities:
Conduct biopsychosocial assessments and develop individualized care plans.
Provide therapeutic counseling and supportive case management.
Collaborate with healthcare professionals, caregivers, and community resources.
Maintain accurate, timely documentation in compliance with agency and regulatory standards.
Participate in interdisciplinary case reviews and team meetings.
Qualifications:
Active DC LGSW or LGPC license required.
Minimum 1 year of experience in behavioral health, community-based care, or related field.
Strong interpersonal, organizational, and communication skills.
Ability to work independently and collaboratively within a team.
Commitment to trauma-informed, patient-centered care.
