RN Case Manager - Home Care in Minneosta
Join us and make a meaningful difference in patients' lives by delivering personalized, compassionate care right in their own homes. As an RN Case Manager, enjoy true autonomy and flexibility while leading interdisciplinary care for diverse patients in the southeast Twin Cities metro area.
Key Responsibilities
- Conduct comprehensive head-to-toe assessments, care coordination, disease/medication management, skilled wound care, catheter/ostomy care, infection prevention, and home safety evaluations.
- Visit patients in their homes to evaluate care quality, review clinical notes, and participate in ongoing interdisciplinary assessments.
- Implement physician-ordered services, educate patients/families on healthcare needs, and complete timely, accurate clinical documentation.
- Collaborate with the healthcare team to ensure seamless coordination, progress tracking, and discharge planning.
- Graduate of an accredited nursing program with current, active RN license in Minnesota.
- Valid driver's license, reliable transportation, auto insurance, and CPR certification.
- Preferred: 2+ years nursing experience, 1+ year case management, community health, and broad med-surg background.
- Competitive salary: $89,419-$106,620 annually.
- Mileage reimbursement or company car program.
- Generous PTO, holiday pay, 401(k) matching.
- Full medical, dental, vision, HSA with company contribution, scrubs allowance, and more.
- 100% clinical license reimbursement; up to $400/year CEU stipend + 2 paid CEU days.
- Full-time, days (Monday-Friday) with holiday rotation (~2/year) and possible weekend rotation based on staffing.
- Field-based role serving patients' homes in southeast Twin Cities metro; office in Roseville, MN.
