Summary
The Restorative/Wound Care Coordinator is responsible for collaborating with the Director of Nursing and Wound Provider and coordinating the delivery of restorative nursing programs and wound care management within the skilled nursing facility. This role ensures high-quality, evidence-based care that promotes resident independence, optimizes functional outcomes, and supports skin integrity and healing. The coordinator serves as a resource, educator, and leader for nursing and therapy staff, while ensuring compliance with Florida regulations, CMS guidelines, and facility policies
Key Responsibilities
Wound Care Oversight
- Assess, monitor, and coordinate individualized wound care plans in collaboration with the interdisciplinary team (IDT), including physicians, nurses, dietitians, and therapists.
- Provide or oversee wound treatments as ordered, ensuring accurate documentation and timely follow-up.
- Serve as a liaison with consulting wound care specialists and physicians.
- Ensure evidence-based practices for the prevention and management of pressure injuries, surgical wounds, diabetic ulcers, and other complex wounds.
- Track, trend, and report wound prevalence/incidence as part of the facility's Quality Assurance and Performance Improvement (QAPI) program.
- Develop, implement, and evaluate restorative nursing programs that promote mobility, self-care, continence, and overall functional independence.
- Coordinate with therapy staff to ensure seamless transition from skilled therapy services to restorative programs.

- Train and mentor nursing assistants in restorative techniques and proper documentation.
- Audit program effectiveness, ensuring interventions are consistent with individualized care plans.
- Provide ongoing staff education on skin integrity, wound prevention, and restorative care best practices.
- Mentor and support CNAs and nurses to maintain high-quality resident care.
- Participate in interdisciplinary care conferences, QAPI, infection control, and regulatory survey readiness activities.
- Ensure documentation meets facility, state, and federal guidelines.
- Maintain knowledge of Florida Department of Health regulations, CMS requirements, and facility policies.
- Collaborate with MDS Coordinator to ensure accurate coding of restorative and wound-related items for reimbursement and compliance.
- Required: Licensed Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed/Registered Physical or Occupational Therapist in the state of Florida.
- Minimum 2 years' experience in long-term care or skilled nursing facility.
- Strong clinical skills in wound management and/or restorative care.
- Knowledge of CMS guidelines, PDPM, and Florida SNF regulations.
- Wound Care Certification (e.g., WCC, WOCN, WCN-C).
- Previous experience as a Restorative Nurse/Coordinator or Wound Care Nurse/Therapist.
- Experience with electronic health record (EHR) systems (Epic, PointClickCare, etc.).
