Home Health LPN (PRN) - Empath Home Health - Naples, FL

Empath Home Health, a part of Empath Health is growing We're expanding our home health services and looking for a compassionate PRN Home Health LPN to join our team in Collier county (Naples, Marco Island, Immokalee, and surrounding areas).

This is an exciting opportunity to serve patients in their homes with the flexibility to set your own schedule and grow with us as we continue to build our presence in the community. In this PRN role, you'll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Locations: Home health visits in Collier county (Naples, Marco Island, Immokalee, and surrounding areas)

Schedule: PRN; 5-10+ home health visits per week.

What You'll Do
  • Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient's plan of care under the direction and supervision of a Registered Nurse.
  • Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
  • Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
  • Performs patient and family instruction on diet, medication, and treatment.
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care
What You'll Need
  • Active Florida LPN license
  • Applicants must be a graduate of an approved nursing program.
  • Minimum of 1-2 years of experience as a LPN.
  • Home health care experience required.
  • Strongly prefers experience using Homecare Homebase (HCHB) EMR
  • Intravenous (IV) Certification preferred.
