If interested in this RN Field Case Manager - Home Hospice position APPLY NOW for IMMEDIATE consideration Brittney Blackman | bblackman@alinestaffing.com | text 5867107970 RN FIELD CASE MANAGER - HOME HOSPICE | DETAILS AND COMPENSATION: Location: Pueblo CO 81008 – 100% On-site Travel required within the Pueblo area Payrate: $56.14/hr + mileage Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM No on-call, weekends, or overtime RN FIELD CASE MANAGER - HOME HOSPICE | SUMMARY AND HIGHLIGHTS: The RN Field Case Manager role supports high-risk members through comprehensive assessments, coordinated care planning, and collaborative communication with healthcare teams. The RN Case Manager ensures patients receive appropriate, cost-effective services while advocating for their safety, privacy, and long-term well-being. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates RN FIELD CASE MANAGER - HOME HOSPICE | RESPONSIBILITIES: Perform comprehensive assessments of high-risk members Collaborate with primary care providers to implement individualized care plans Identify and reassess cost-efficient and clinically appropriate levels of care Coordinate care transitions and support non-medical needs (housing, transportation, etc.) Work closely with physicians, social workers, discharge planners, and claims teams Engage specialty resources when needed to achieve optimal outcomes Maintain detailed documentation of clinical, functional, and fiscal progress Promote health education, illness prevention, and patient safety Act as a patient advocate while upholding privacy and confidentiality standards RN FIELD CASE MANAGER - HOME HOSPICE | REQUIREMENTS: Education: Associate’s or Bachelor’s Degree in Nursing Attendance: Commitment to mandatory attendance during the first 90 days. Current, unrestricted RN license (CO or multi-state) Case Management certification preferred Ability to analyze complex clinical information and make sound decisions Reliable transportation and active auto insurance Travel required within the Pueblo area Preferred: Experience with HomeCare HomeBase (HCHB) and/or hospice care Take the Next Step Join a company that values your growth and contributions.
