About the Role:

The Health Services Coordinator is vital in ensuring members receive tailored and ongoing healthcare services that meet their unique needs. This role revolves around collaborating with healthcare providers, members, and families to create, implement, and monitor personalized care plans that lead to optimal health outcomes. Acting as a liaison, the Coordinator facilitates communication among multidisciplinary teams to ensure seamless and effective care delivery. By identifying and addressing potential barriers to care proactively, the Coordinator plays a crucial role in reducing hospital readmissions and increasing member satisfaction. This position is essential in improving the quality of care and supporting members throughout their healthcare journey.

Minimum Qualifications:

  • High School Diploma or GED
  • At least 2 years of experience in care coordination, case management, or a related healthcare role.
  • Strong understanding of healthcare systems, member care processes, and community resources.
  • Proficiency in electronic health records (EHR) and basic computer applications.

Preferred Qualifications:

  • Associate's degree in nursing, Social Work, Healthcare Administration, or a related field.
  • Certification in Case Management (CCM) or Care Coordination.
  • Experience working with diverse member populations and knowledge of cultural competency in healthcare.
  • Familiarity with insurance processes, billing, and healthcare regulations such as HIPAA.
  • Bilingual abilities to support non-English speaking members.

Responsibilities:

  • Assess member needs and collaborate with healthcare professionals to develop individualized care plans.
  • Schedule appointments, manage referrals, and ensure timely delivery of services and follow-up care.
  • Act as the primary contact for members and their families, providing education and guidance on resources.
  • Work with clinical teams, community partners, and internal departments to deliver cohesive care.
  • Document care activities accurately in compliance with organizational policies and healthcare regulations.
