Pay Rate: $30 per hour (W2)
Job Description:
We are seeking an experienced and dedicated Case Worker to provide essential social services within a healthcare setting. The Case Worker will support patients and families by identifying needs, developing service plans, determining eligibility for assistance programs, and offering ongoing counseling and support. This role involves close interaction with individuals receiving public assistance, medical assistance, or protective services and requires strong assessment and communication skills.
Responsibilities:
- Provide social services to patients and clients in need of public assistance, food stamps, medical assistance, protective services, or institutional care.
- Conduct assessments to determine client needs and eligibility for available services.
- Develop, implement, and monitor individualized social service plans.
- Perform counseling and investigatory activities to support client well-being and ensure compliance with program requirements.
- Collaborate with healthcare staff, community agencies, and other support professionals to coordinate care.
- Maintain accurate documentation, case notes, and reports in accordance with regulatory standards.
- Advocate for clients and assist them in navigating community resources and support programs.
- Uphold professional standards, confidentiality, and all compliance guidelines.
- Minimum of 3 years of relevant experience in case work, social services, or a related field.
- Strong communication, assessment, and problem-solving abilities.
- Ability to work with diverse populations and manage sensitive situations with empathy and professionalism.
- Baccalaureate Degree from an accredited college or university.
