Role Responsibilities
- Deliver thorough, continuous healthcare for individuals and families throughout every stage of life, from childhood to eldercare.
- Address chronic illnesses, focus on preventive measures, manage acute conditions, and attend to mental health needs through a patient-first lens.
- Perform physical assessments, analyze diagnostic outcomes, and draft tailored treatment strategies.
- Work in tandem with care coordinators, mental health specialists, and other experts to ensure comprehensive patient care.
- Engage in initiatives aimed at enhancing quality and outcomes for patients and operational performance.
- Utilize electronic medical records (EMR) systems to maintain precise, current patient documentation.
- Provide care both in person and through virtual platforms to meet the varying needs of patients.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
- Completion of a Family Medicine residency program accredited by a recognized body.
- Board Certification in Family Medicine.
- Eligible for, or currently possess, an active medical license in Pennsylvania.
- Preferred certifications in BLS and ACLS.
- Exceptional diagnostic acumen with a strong focus on preventive healthcare and proactive patient engagement.
- Familiarity with value-based healthcare models and an eagerness for continuous improvement.
- Excellent interpersonal communication skills complemented by a collaborative spirit.
- Technologically adept, with experience utilizing EMRs and telemedicine tools.
