PACE Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.

Position Summary
The Member Services Coordinator (PACE) provides administrative and coordination support to clinical and interdisciplinary teams. This role ensures efficient scheduling of external appointments, maintains accurate scheduling and medical records, and facilitates communication with internal and external stakeholders. The position plays a critical role in supporting care delivery by coordinating transportation, verifying appointment details, and following up on required documentation.
Essential Functions

  • Maintain and update participant rosters with relevant information.
  • Review, process, and schedule provider orders in a timely manner.
  • Update electronic medical records (EMR) with appointment details and statuses.
  • Create service authorizations for approved clinical services.
  • Coordinate transportation and escort arrangements with relevant team members.
  • Prepare and distribute weekly schedules of external appointments.
  • Communicate appointment details to participants and/or caregivers.
  • Confirm appointments with external providers and ensure all required documentation is received.
  • Retrieve medical records from external specialists and facilities post-appointment.
  • Monitor schedule changes and address discrepancies.
  • Follow up with clinical staff regarding completed appointments and potential follow-ups.
  • Audit appointment compliance and address discrepancies.
  • Support quality improvement initiatives.
  • Ensure compliance with all applicable state and federal regulations, including timely access to services and proper grievance procedures.
  • Protect privacy and confidentiality of participant and agency information.
  • Perform other duties as assigned.
QualificationsKnowledge and Critical Skills
  • Effective written and oral communication skills
  • Excellent customer service and interpersonal skills
  • Strong problem-solving and organizational abilities
  • Ability to prioritize tasks and meet deadlines independently or within a team
  • Proficiency in Microsoft Office Suite and healthcare data management systems
Experience
  • Minimum 1 year of customer service experience
  • At least 1 year of experience working with frail or elderly populations, or willingness to complete required training upon hire
  • Light secretarial skills and professional telephone etiquette
License / Certification
  • Certified Medical Assistant, preferred
Education
  • High school diploma or equivalent
  • Certified Medical Assistant (CMA) certification, preferred

  • Must meet competency standards established by the organization before working independently
