Home Health Registered Nurse - NW Portland, OR

Location

Northwest Portland, Oregon and surrounding areas

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5 days a week, 8-hour shifts (08:00 AM - 04:30 PM). This facility cannot accommodate 4x10s or 3x12s.

Job Description

The Home Health Registered Nurse will be responsible for providing direct patient care in a home health setting. The nurse will visit patients in their homes to assess their health needs, develop care plans, and manage ongoing medical treatment.

Responsibilities

  • Perform admission assessments and routine visits for home health patients.
  • Develop and implement individualized care plans based on patient needs.
  • Provide education and support to patients and their families on disease management and care techniques.
  • Manage medication administration, including G-tube and PCA pump management.
  • Conduct pain assessments and utilize pain management strategies.
  • Document patient care accurately using the Oasis and EPIC charting systems.
  • Coordinate care with interdisciplinary teams to ensure patient-centered outcomes.
Required Experience / Certifications / Licensure
  • Minimum of 1 year of skilled home health experience.
  • Current BCLS certification is required.
  • Experience with Oasis documentation is essential.
  • Must have a valid Oregon state nursing license or be pending license acceptance.
  • Reliable transportation is required to visit patients within a 60-mile radius.
  • COVID vaccination (primary series plus one booster) is required.
