The primary purpose of theTransporter position is to provide each assigned resident with excellent dailynursing care and services to assure resident safety in accordance with theresident's assessment and plan of care, and as may be directed by your supervisor. Under directsupervision, performs duties necessary to provide safe, courteous and efficienttransport of patients, equipment, supplies and specimens to and from patientcare areas and departments within the facility or campus assigned.
JOB DUTIES &RESPONSIBILITIES: AdministrativeFunctions
• Identifyresidents before administering treatments, serving meals, etc. as necessary.
• Recordall entries on flow sheets, notes, charts, etc. in an informative anddescriptive manner.
• Useonly authorized abbreviations established by this facility when recordinginformation.
• Reportall changes in the resident's condition to the RN Team Leader as soon aspossible.
• Reportand provide statement for all events, accidents and incidents you observe onthe shift as they occur.
Admission, Transfer, and Discharge Functions
• Transports patients by assisting patients in andout of taxis, ambulances, and cars; lifting patients on and off beds; movingthem to and from special service and treatment areas; using wheelchairs ormoveable beds.
• Transports equipment and supplies; deliveringthem to assigned treatment areas and patient care areas.
• Transports laboratory specimens by picking-upspecimens and delivering them to specified laboratory.
• Safely transport patients to and from eachlocation by following all safety protocols/processes.
• Retrieves and stores transport equipment.
• Uses wheelchairs and other approved rollingstock to carry out the essential duties of the position.
• Demonstrates competency in transporting allspecific age groups including the use of patient lift equipment.
• Ensures the safe, timely and accurate transportof patients to their destination using the proper requested equipment.
• Communicates effectively with nursing personnel,clinicians, technicians or reception and dispatch.
• Ensures the comfort of the patient by beingattentive. Offers blankets, checks modesty and only engages in appropriateconversation involving patients.
• Alerts nursing staff immediately to any signs ofpatient distress or equipment malfunction.
• Maintains equipment and reports equipmentneeding repairs.
• Complies with regulatory agency standards,including federal, state local governmental regulations.
• Adheres to facility confidentiality andpatient's rights policy as outlined in the facilities HIPPA policies andprocedures.
• Performs other duties as assigned.
Completes all necessary deliveryforms and paperwork including, but not limited to invoices, work orders,manifests and logs, patient care instructions, route sheets, etc.
Delivers equipment and oxygen as required inaccordance with industry standards and applicable federal, state and localgovernmental regulations.
• Delivers, sets up and maintains equipment at thedelivery address using company vehicle.
• Educates the patient and/or caregiver on thesafe use and maintenance of the delivered items.
• Maintains assigned company vehicle in a cleanand safe working condition.
• Observes additional needs that the patient mayhave and reports to management during morning meeting
• Reports equipment hazards and/or productincidents as required
• Ensure that the resident's room is ready forreceiving the resident (i.e., bed made, name tags up, admission kit available,etc.)
• Greet residents and escort them to their room.
• Introduce resident to his/her roommate, if any,and other residents and personnel as appropriate.
• Make resident comfortable (i.e., put to bed, getwater, etc.) • Inventory and mark the resident'spersonal possessions as instructed.
• Store resident's clothing.
• Assist residents with packing their personalbelongings when they are being transferred or discharged.
• Transport residents to new rooms or to thereceiving area.
• Assist with loading/unloading residents fromvehicles as necessary.
Personnel Functions
• Performall assigned tasks in accordance with our established policies and procedures,and as instructed by your supervisor.
• Followwork assignments, and/or work schedules in completing and performing yourassigned tasks.
• Cooperatewith inter-departmental personnel, as well as other facility personnel toensure that nursing services can be adequately maintained to meet the needs ofthe residents.
• Createand maintain an atmosphere of warmth, personal interest and positive emphasis,as well as a calm environment throughout the unit and the shift.
• Meetwith your shift's nursing personnel, on a regularly scheduled basis, to assistin identifying and correcting problem areas, and/or the improvement of servicesand participate in Team Conference.
• Reportall complaints and grievances made by the resident.
• Notifythe facility when you will be late or absent from work.
Personal Nursing Care Functions
• Participate in and receive the nursing reportupon reporting for duty.
• Follow established policies concerning exposureto blood/body fluids.
• Assist residents with hygiene needs to include:
• Daily dental and mouth care (i.e. brushingteeth/dentures, oral hygiene, special mouth care, etc.)
• Bathing (i.e. bed bath, tub or shower bath,etc.) as directed
• Dressing/undressing as necessary
• Hair care needs (i.e. combing, brushing,shampooing, etc.)
• Nail care (i.e. clipping, trimming and cleaningthe finger/toe nails)
• Shaving male residents
• Controlling hair growth on female residents(i.e. facial hair, under arms, on legs, etc.) as instructed • Keepingresidents dry (i.e. change gown, clothing, linen, etc. when they become wet orsoiled)
• Change bed linens. Keep linens tight to avoidwrinkles from forming under the resident. • Make beds (occupied andunoccupied.) • Put extra linens on bed as requested.
• Position bedfast residents in correct andcomfortable position, utilizing positioning devices as indicated.
• Assist resident with bowel and bladder functions(i.e., take to bathroom, offer bedpan/urinal, portable commode, etc.)
• Maintain intake and output records asinstructed.
• Keep incontinent residents clean and dry.
• Check and report bowel movements and characterof stools as instructed.
Assist residents in preparing formedical tests and appointments (i.e., lab work, x-ray, therapy, dental,physician dialysis, etc.)
Assist residents in preparing foractivity and social programs (i.e. church, parties, visitors, physician,dialysis).
• Assist in transporting residents to/fromappointments, activity and social programs, etc. as necessary.
• Assist with lifting, turning, moving,positioning, and transporting residents into and out of beds, chairs, bathtubs,wheelchairs, lifts, etc.
• Assist residents to walk with or withoutself-help devices as instructed.
• Provide eye and ear care (i.e., warm/coldcompresses, removing wax and hair, cleaning eyeglasses/hearing aids, dentures,etc.) as instructed.
• Assist in preparing the resident for a physicalexamination.
• Measure and record temperatures, pulse, andrespirations, as instructed.
• Answer resident calls promptly.
• Check each resident routinely to ensure thathis/her personal care needs are being met in accordance with his/her wishes.
• Assist with the care of the dying resident asdirected.
• Provide post-mortem care as instructed.
Special Nursing Care Functions
• Observeand report the presence of red areas and skin breakdowns to prevent decubitusulcers.
• Reportinjuries of an unknown source, including skin tears.
• Providedaily indwelling catheter care.
• Providedaily perineal care.
• Performnon-sterile (clean) vaginal irrigation/douches.
• Assistwith the application of slings, elastic bandages, binders, etc.
• Givetepid sponge baths.
• Providedaily range of motion exercises and record data as instructed.
• Turnbedfast residents at least every two (2) hours.
• Performdiabetic urine testing, as directed.
• Observedisoriented and comatose residents. Record and report data as instructed.
• Turnall medications found in the resident's room over to the MedicationAdministration Manager.
• Watchfor and report any change in room temperature, ventilation, lighting, etc. tothe RN Team Leader.
Food Service Functions
• Prepareresidents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed,position tables, place clothing protectors, take to/from dining room, etc.)
• Servefood trays. Assist with feeding as indicated (i.e., cutting foods, feeding,assist in dining room supervision, etc.)
• Assistresidents with identifying food arrangements (i.e. informing resident withsight problem of foods that are on his/her tray, where it is located, if it ishot/cold, etc.)
• Recordthe residents' food intake. Report on changes in the resident's eating habits.
• Keepresidents' water pitchers clean and filled with fresh water and within easyreach of the resident.
• Servebetween meal and bedtime snacks.
• Performafter meal care (i.e. remove trays, clean resident's hands, face, clothing,take to bathroom, brush teeth, clean dentures, etc.)
• Checkrooms for food articles (i.e. food in proper container, unauthorized fooditems, etc.)
• Maintainintake/output records as assigned.
Safety and Sanitation
• Applyrestraining device to residents in chair/bed as ordered.
• Checkresidents with restraining device at least every thirty (30) minutes or asordered.
• Releaserestraining device at least every two (2) hours for range of motion exercises,taking to bathroom, etc.
• Maintaina record of residents with a restraining device, the time devices werereleased, and how long the restraining device was released.
Participate in appropriatein-service training programs prior to performing tasks that involve potentialexposure to blood/body fluids.
Wash hands before and afterperforming any service for the resident.
• Keepthe nurses' call system within easy reach of the resident.
• Immediatelynotify the RN Team Leader of any resident leaving/missing from the facility.
• Followestablished safety precautions in the performance of all duties.
• Keepresidents' personal possessions off the floor and properly stored.
• Keepfloors dry. Report spills immediately.
• Keepexcess supplies and equipment off the floor. Store in designated areas.
• Clean,disinfect, and return all resident care equipment to its designated storagearea after each use.
• Reportall hazardous conditions and equipment to the RN Team Leader immediately.
• Reportall safety violations.
• Followestablished smoking regulations. Report all violations.
• Reportany communicable or infectious disease to the DON and/or the Infection ControlCoordinator.
• Followestablished isolation precautions and procedures.
• Washhands before entering and after leaving an isolation room/area.
• Followestablished procedures in the use and disposal of personal protectiveequipment.
• Wearand/or use safety equipment and supplies (i.e. back brace, mechanical lifts,etc.) when lifting or moving residents.
• Reportmissing/illegible labels and MSDSs to your supervisor.
Equipment and Supply Functions
• Useonly the equipment you have been trained to use.
• Operateall equipment in a safe manner.
• Reportdefective equipment to the RN Team Leader.
• Informthe RN Team Leader of your equipment and supply needs.
Care Plan Functions
• ReviewCNA care cards daily to determine if changes in the resident's daily careroutine have been made on the care plan.
• Informthe RN Team Leader of any changes in the resident's condition so thatappropriate information can be entered on the resident's care plan.
• Ensurethat your notes/documentation reflect that the care plan is being followed.
Resident Rights Responsibilities
• Maintainthe confidentially of all resident care information.
• Ensurethat you treat all residents fairly, and with kindness, dignity, and respect.
• Ensurethat all nursing care is provided in privacy.
• Knockbefore entering the resident's room.
• Reportall complaints made by the resident and/or family member to the RN Team Leader.
• Reportall allegations of resident abuse and/or misappropriation of resident propertyimmediately.
• Honorthe resident's refusal of treatment request. Report such requests to the RNTeam Leader.
Working Conditions

• Issubject to callback during emergency conditions.
• Attendsand participates in continuing education programs.
Is subject to injury from falls,burns from equipment, odors, etc., throughout the workday, as well as toreactions from dust, plastics, synthetics, disinfectants, tobacco smoke, andother potential contaminants.
• Issubject to exposure to infectious waste, disease, conditions, etc. including TBand the AIDS and Hepatitis B viruses.
• Maybe subject to the handling of and exposure to hazardous chemicals.
Staff Development Functions
• Attendand participate in scheduled training and educational classes to improvenursing skills and maintain current certification as a Nursing Assistant.
• Attendand participate in scheduled orientation programs and activities.
• Attendand participate in annual OSHA and CDC in-service training programs for hazardcommunication, TB management, and blood borne pathogens standards.
• Attendand participate in advance directive in-service training programs as scheduled.
