Key Responsibilities:
- Assess the nutritional needs of residents and develop comprehensive care plans documented within the electronic medical record.
- Create, implement, and review individualized nutrition care plans based on clinical findings, lab results, weight trends, intake, and comorbidities.
- Deliver nutrition education and counseling to residents, families, and staff.
- Collaborate with interdisciplinary teams to enhance residents' overall health and well-being.
- Ensure compliance with CMS, State, and Federal regulations and company standards.
- Provide coverage to multiple facilities based on company needs and preferences.
- Perform all other assigned duties.
Qualifications:
- Registered by the Commission on Dietetic Registration. **
- Valid Certification/License in good standing for the relevant state.
- Preferred but not required: Knowledge of the state survey process and compliance regulations.
- Strong organizational and communication skills.
- Positive attitude and ability to communicate effectively with residents, administration, and HCSG leadership.
- Compliance with COVID-19 vaccination policies as required by state and local regulations.
- If a multi-site specialist, must possess a valid Driver's License.
**RD-Eligible and provisional state licensure accepted in certain locations.
