Case Manager - Day Shift

This is a 13-week contract position.
Schedule
Day shift, Monday to Friday.
Job Description
The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation.
Responsibilities
- Identify patient problems affecting treatment participation and assist in utilizing hospital resources.
- Collaborate with the interdisciplinary team to direct a clinical plan of care and facilitate safe discharge plans.
- Ensure accurate documentation and compliance with safety policies.
- Conduct team conferences and submit professional reports.
- Review and manage medical necessity as mandated by the Quality Improvement Organization (QIO).
- Maintain community resource knowledge and assist families in accessing appropriate services.
- Optimize outcomes through effective management of utilization reviews and unnecessary services.
- Current licensure in a clinical discipline per state guidelines (RN, RT, SW preferred).
- Working knowledge of the insurance industry and government reimbursement.
- Two years of experience in Case Management and Discharge Planning in an Acute Care setting.
