JCCA's Youth Assertive Community Treatment (ACT) is a program designed to address the significant needs of youth ages 10 to 21, who are at risk of entering, or returning home from, high end services, such as inpatient settings or residential services, through the use of a multi-disciplinary team. Children with significant psychiatric needs, who are at risk of institutional level of care, require intensive interventions in order to adequately support the child and family's complex needs, to avoid high end services or facilitate and support a successful transition back to community.
JCCA's Youth ACT programs serve as a critical component in the children's continuum of care and ensures the child and their family have the level of support services and access to clinical professionals they require to sustain any gains made in crisis response or high-end services. Team interventions are focused on improving or ameliorating the significant functional impairments and severe symptomatology experienced by the child/youth due to mental illness or serious emotional disturbance (SED). Interventions are also focused on enhancing family functioning to foster health/well-being, stability and re-integration for the child/youth. Services are delivered using a family-driven, youth guided and developmentally appropriate approach that comprehensively addresses the needs of the child/youth within the family, school, medical, behavioral, psychosocial, and community domains. Youth/families enrolled in ACT are the responsibility of the collective team, and not just one or two individuals on the team.
Position Summary:
The Licensed Mental Health Professional is responsible for providing treatment to children and their families to support identified treatment goals and targeted service interventions. They will address the clinical needs of the children and the complex needs of the entire family unit. Treatment interventions utilized by this team member must be individualized to the child and family, as well as evidence-based. This role will focus on a strengths-based approach and work with the rest of the Youth ACT Team to support all parties involved. They will assist with providing therapeutic communication and interactions with youth/families for the purpose of alleviating symptoms or functional limitations associated with a child/youth's behavioral health diagnosis, reversing or changing maladaptive patterns of behavior, encouraging personal growth and development, and supporting the child/youth's capacity to achieve age-appropriate developmental milestones.
Responsibilities:
- Provide case contacts - individual/family/group therapy, assist in formulating diagnoses, assessments, treatment planning, monitoring, crisis management, crisis response - 75% of time
- Participation in team meetings/supervision - 10% of time
- Documentation - 15% of time
- On-call-share on-call responsibilities with the Team Lead and Assistant Team Lead on a rotating basis, where the Licensed Mental Health professionals respond to crisis calls after hours and on weekends. Additional compensation provided for on-call rotations.
- The successful candidate must have one of the following credentials from NYS:
- Licensed Master's Social Work (LMSW)
- Licensed Marriage Family Therapist (LMFT)
- Licensed Mental Health Counselor (LMHC)
- Must have at least three (3) years of experience working with children and youth with significant emotional disorders, severe mental, emotional and behavioral impairments (commensurate with RTF or CR level of care), histories of hospitalization, and families with complex, multi-system needs
- Experience with providing evidence-based treatments and familiarity with evidence-based practices is crucial to this role
- Excellent Computer skills (particularly Microsoft Office, Excel, Word and PowerPoint)
- Knowledge of Managed Care Organizations (MCOs), Medicaid and Federal guidelines
- Demonstrated knowledge of evidence based models and practice
- Demonstrated knowledge of Social Determinants of Health, Race Equity, Social Justice, and Trauma Informed care
- Must demonstrate the ability to collaborate with client and teams that are culturally and professionally diverse
- Fluency in Spanish is highly preferred
Salary Range: $62,400 - $64,900 + Differential added for Spanish language skills and credentials
