The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
GENERAL PURPOSE:
The Senior Social Worker assigned to Pediatric Behavioral Health & Wellness (PBH&W): Brighter Days Urgent Care Center functions as the Admission and Disposition Coordinator and provides clinical supervision to START clinicians and Walk-In Clinicians. As Admission and Disposition Coordinator, the Senior Social Worker collaborates within URMC and with other community agencies to coordinate admissions and engage in disposition planning for patients presenting to the Urgent Care. The Senior Social Worker also provides direct clinical care as a Walk-In Clinician and START clinician. In the role of a Walk-In clinician, the team member provides an initial mental health/safety assessment, and immediate evidence-informed crisis intervention, as well as follow-up recommendations and linkage to appropriate community resources and treatment (e.g., START). In the role of a START clinician, the team member provides short-term solution-focused treatment to patients and their families and is responsible for assessment, crisis intervention, treatment planning, and therapy in the up to 6-session model of care.
The Sr. SW will support the coordination of the Intercept program. This coordination includes the management of potential referrals for the program, outreach to provide program education, screening of patients to determine appropriateness for the service. This may also include attending/facilitating weekly team meeting, and training/ onboarding new clinicians on the program specific interventions and procedures.
ESSENTIAL FUNCTIONS

Provide direct patient care to youth and their families in the Walk-In track of the PBH&W Brighter Days Urgent Care Center.

More specifically, provides an initial mental health/safety assessment, and evidence-informed crisis intervention, as well as follow-up recommendations and linkage to appropriate community resources and treatment.

INTERCEPT program coordination which includes outreach to potential patients/families, referral management and triage.

Screen patients/referral appropriateness prior to scheduling appointment, including outreach to previous and current providers.
-
Schedule clinical appointments and communicate referral status with clinicians.

Maintain database of program patients and generate weekly reports of caseloads.

Train/onboard new clinicians on program specific interventions and procedures.

Provides administrative, discipline, and clinical supervision to social workers, staff clinicians and trainees.

Shares responsibility for the quality of care being provided by the supervisee (e.g., shares knowledge of evidence-based assessment and interventions, community resources, models ethical behavior and standards, provides regular feedback on strengths and areas for growth, communicates with other members of the supervisory team).

Supervisors provide oversight to ensure that supervisees meet service standards (e.g., clinical productivity and timely completion of high-quality documentation).

Participates in on-call activities (on a rotating basis) of the program area, including after hours.

Documents treatment in patients' electronic medical record in compliance with Program, Department and Hospital regulations.

This includes the development of family centered, comprehensive and individualized treatment plans for patients with clearly defined therapeutic goals with patients and families; ensures that Social Work Division, Department, Program, and Hospital policies and procedures are implemented with regard to professional practice, patient care and discharge planning.

INTERCEPT specific outreach to sites of potential referrals (e.g. College counseling centers, child and adolescent psychiatry clinics, pediatric practices.) to provide education about Intercept, which would include presentations, as applicable.

Work to expand referral network in Buffalo, Rochester, Syracuse.

Track referral source information. Perform other administrative tasks, as needed.

Coordinates Admission and Disposition planning within Urgent Care, including planning and program development.

Collaborates with triage and referral sources and other professionals both internal and external (ex. Community outpatient services, PCPs, Schools, Mobile Crisis Team, CPEP, Crisis Call Line, etc.).

Provide direct patient care to youth and their families in the START (Short-Term Assessment & Response Team) track of the PBH&W Brighter Days Urgent Care Center under general direction and with considerable latitude for independent judgement.

More specifically, provides short-term solution-focused treatment to patients and their families and is responsible for assessment, crisis intervention, treatment planning, and therapy in the up to 6-session model of care. Mobilizes patient, family, and natural support networks when clinically indicated. Meets minimum target for clinical productivity.

Recruits and evaluates, in collaboration with the clinical coordinator and program director, the performance of social workers assigned to the PBH&W Brighter Days Urgent Care Center service including quarterly audits of electronic medical records.

Coordinates appropriate referrals and linkage to mental health services as clinically indicated.

Establishes and regularly maintains cooperative working relationships with referral sources and other professionals both internal and external throughout patient care activities (e.g., PCPs, community mental health agencies). Coordinates with professional and administrative staff of the Medical Center and University as well as support staff regarding patient scheduling, fees, and insurance issues.

Attends Social Work Division, Social Work in Psychiatry and Program staff meetings.

Participates in activities of the Social Work and Psychiatry Division Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented with regard to professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.

Participates in routine committee process of the Social Work Division and facilitates Division and Hospital quality assurance and staff development activities.

Subscribes to the departmental goal of creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

Participates in available cultural competency events and training appropriate to job duties.

Maintains professional CEU requirements by reading, attending seminars, workshops and conferences.
QUALIFICATIONS

Master's degree from an accredited school of social work (CSWE). Required
-
2 years professional social work practice beyond the Master's degree.

Experience providing clinical services to children with behavioral health disorders and their families. Required

1-2 years' experience with clinical supervisory experience. Preferred
