Schedule: Day Shift | 8:00 AM - 4:30 PM
Location: Pleasant Hill, CA (Floating required between two nearby facilities)
Position Overview
We are seeking a licensed Occupational Therapist (OT) for a 13-week contract assignment. The therapist will primarily work Monday through Friday, with occasional weekend coverage as needed. This position includes floating responsibilities to an affiliated nearby facility based on operational requirements.
Key Responsibilities
- Provide high-quality occupational therapy services to patients in a post-acute/rehabilitation setting.
- Conduct evaluations and develop individualized treatment plans to improve functional abilities.
- Deliver therapeutic interventions focusing on ADLs, mobility, strength, coordination, and patient independence.
- Document assessments, progress notes, care plans, and discharge summaries in accordance with facility and regulatory guidelines.
- Collaborate with an interdisciplinary team including PTs, SLPs, nurses, and care coordinators.
- Participate in care conferences and contribute to patient-centered goal setting.
- Float between assigned facilities based on staffing needs.
- Provide weekend coverage as scheduled or required.
- Active and unrestricted Occupational Therapist license in the state of assignment.
- Minimum of 1 year of OT experience preferred (SNF experience a plus).
- Strong clinical reasoning and patient-centered care approach.
- Ability to work independently and within a multidisciplinary team.
- Reliable and flexible to meet weekday, weekend, and floating requirements.
