Social Worker Heritage Home Health

Responsible for providing medical social services to promote physical, emotional, spiritual, and psychosocial well-being of the hospice patient. This is performed in accordance with physician orders and patients plan of care under the direction of the Administrator.
Responsibilities:
* Understands and adheres to established Agency policies and procedures.
* Participates in peer review and Quality Assessment and Performance Improvement as assigned.
* Participates in Interdisciplinary Group meetings.
* Completes and submits assessments and reassessments in condition in accordance with Agency defined time frames.
* Comply with all applicable Federal and State laws and regulations, including HIPAA.
* Supportive of individual and company performance improvement initiatives
* Maintain confidentiality with patient, employee and company sensitive information
* Comply with accepted professional standards and practices
* Perform other duties as assigned by the Supervisor
Knowledge, Skills and Abilities: Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; OR A baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; OR A baccalaureate degree in social work, psychology, sociology, or other field related to social work and is supervised by a MSW meeting qualification above; AND At least one (1) year of social work experience in a health care setting. Current Social Work license in good standing with the board in the state of practice. Knowledge/experience of Home Health strongly preferred. Must have knowledge of current Medicare, Medicaid, State and Federal regulations.
