Job Summary:
Conducts health assessment interviews focused on wound, ostomy, and continence-related care. Performs focused physical assessments, develops plans of care based on diagnosis and individual client needs, and recommends medications and treatments as appropriate.

Job Responsibilities:
  • Develop, revise, and evaluate standards of care; assist in the selection of supplies, technology, and equipment.
  • Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wound treatments.
  • Evaluate the quality and delivery of care provided by clinical staff on the unit.
  • Work collaboratively with the healthcare team and make referrals to other disciplines as indicated.
  • Teach, counsel, and assist individuals and families in assuming responsibility for preventing complications related to ostomies, wounds, or continence issues.
  • Develop, provide, and evaluate staff education programs (e.g., new employee orientation, in-services, continuing education).
  • Routinely provide comprehensive assessments of patients at risk for pressure injuries and initiate preventive measures.
  • Escalate and communicate challenges identified, including ethical issues and conflict resolution.
  • Collaborate routinely with the Assistant Director of Nursing for Wound Care to initiate and recommend best practices based on assessment.
  • Participate in Unit/Department Quality Improvement (QI) and Hospital-wide QI activities (e.g., Incidence and Prevalence Studies, Nursing Quality).
  • Provide culturally competent education to patients, families, and staff consistent with cultural, ethnic, religious, and belief systems.
  • Maintain security and confidentiality of data and information in accordance with patient rights policies.
  • Achieve and maintain professional competency by engaging in activities consistent with clinical practice and population served.
  • Identify and assist clients in utilizing available community resources.
  • Manage assets related to rented equipment (e.g., specialty beds, negative pressure devices).
Required Skills & Experience:
  • Minimum of two (2) years of Med/Surg experience.
  • Experience in wound and colostomy care.
Required Education:
  • Associate Degree in Nursing (ASN, ADN) from an accredited nursing program

    Required Certifications & Licensure:
  • New York State Registered Nurse (RN) License.
  • Primary Source Verification of License and Registration as a Registered Professional Nurse from the NYS Department of Education (NYSED).
  • American Heart Association (AHA) Basic Life Support (BLS) certification.
  • Current certification in one of the following:
    • Certified Wound Ostomy Continence Nurse (CWOCN) - Wound, Ostomy and Continence Nursing Certification Board (WOCNCB)
    • Certified Wound Ostomy Nurse (CWON) - WOCNCB
    • Certified Wound Care Nurse (CWCN) - WOCNCB
    • Certified Wound Specialist (CWS) - American Board of Wound Management (ABWM)
    • Wound Care Certified (WCC) - National Alliance of Wound Care and Ostomy (NAWCO)
