Home Health / Hospice Nurse Job Description
- Receive patient assignments from the Operations Leader or Clinical Resource Team Leader.
- Communicate effectively regarding patient care, business operations, and topics that foster a positive work environment.
- Assume responsibility for patient and family care, including assessment, planning, implementation, and evaluation.
- Initiate and maintain communication with attending physicians, hospice staff, and other agencies to ensure optimal patient care and resource utilization.
- Collaborate regularly with the Operations Leader or designated personnel to review care strategies.
- Maintain consistent communication with attending physicians about patient and family care.
- Collect and integrate data on the physical, psychological, social, and spiritual factors influencing patient and family health into the care plan.
- Minimum of two years of nursing experience preferred, with at least one year in home care highly desirable.
- Current BLS Certification.
- Valid Registered Nurse licensure in South Carolina.
- Valid driver's license and proof of vehicle insurance.
