Position Overview
As the Nurse Care Coordinator, you'll play a pivotal role in managing daily nursing operations, mentoring staff, and ensuring the highest standards of resident care. Acting as a key liaison between leadership, nursing teams, residents, and families, you'll help drive excellence across every aspect of care delivery.
Key Responsibilities
- Coordinate, supervise, and guide nursing service staff to ensure consistent, high-quality care.
- Support administrative and clinical functions in the absence of the D.O.N., A.D.N., or Executive VP.
- Ensure departmental compliance with policies, procedures, and regulatory standards.
- Serve as a role model and clinical resource to nursing personnel.
- Foster effective interdepartmental communication and teamwork.
- Participate in nursing and administrative committees to enhance operational performance.
- Identify training needs, conduct competency assessments, and support staff education.
- Oversee staffing schedules and assist in performance evaluations.
- Build positive relationships with residents and families to promote comfort, satisfaction, and well-being.
Requirements
- BSN required; MSN preferred.
- Current NYS Registered Nurse license in good standing.
- Proven experience as an RN Supervisor, Head Nurse, or Care Coordinator in a long-term care or rehabilitation setting.
- Comprehensive knowledge of nursing principles, LTC standards, and patient care techniques.
- Strong leadership, communication, and mentoring abilities.
- Experience with SigmaCare and EPIC preferred.
