About the Role
In this role, you will conduct comprehensive assessments during visits that last up to one hour. Our assessments cover a member's health, medical history, medication adherence, and social environment, providing insights essential for improving care quality. Your responsibilities will include:
- Conducting adult and geriatric assessments, including documentation according to Matrix guidelines
- Administering and ordering lab-based and point of care testing as necessary
- Collaborating with a multidisciplinary team, including physicians and social workers
- Educating members and coordinating follow-up care with their Primary Care Physician
- Providing services in members' homes or Skilled Nursing Facilities

Skills & Experience That Shine
- Master's Degree or equivalent experience with successful NP or PA licensure
- Current NP or PA licensure with prescriptive authority in the state of practice
- Board certification in a relevant specialty (e.g., Adult, Family, Emergency)
- Current BLS, ACLS, or CPR certification
- Preferred but not required: 1 year of experience as a Nurse Practitioner or Physician Assistant; new graduates are encouraged to apply
- Valid driver's license and proof of auto insurance
- Strong computer skills and familiarity with health/medical record software
- Excellent verbal and written communication skills
