Travel Physical Therapist - Home Health - Charlotte, NC
13-Week Contract | Up to $2,700 Weekly (Based on Experience)
Connected Health Care, a leading recruitment and staffing firm, is seeking an experienced and compassionate Travel Home Health Physical Therapist to join one of our trusted healthcare partners in Charlotte, North Carolina. This position offers the perfect blend of professional growth and work-life balance in one of the South's most vibrant and livable cities.
Contract Length: 13 Weeks
Shift: Day Shift
Weekend Rotation: None
On Call: Not required (unless requested)
Job Description:
  • Evaluate and assess patients' physical therapy needs within their home environment
  • Develop individualized treatment plans to improve mobility, strength, balance, and independence
  • Provide hands-on therapy, education, and home exercise instruction tailored to patient goals
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
  • Monitor progress, adjust plans as needed, and document all care provided
  • Utilize critical thinking and communication skills to deliver compassionate, patient-centered care
  • Travel to patients' homes in the local area to provide high-quality physical therapy services
Education and Certification Requirements:
  • Active North Carolina Physical Therapist (PT) License required
  • AHA BLS Certification required
  • Minimum of 1 year of Physical Therapy experience required
  • Must have recent Home Health experience
  • First-time travelers accepted if seasoned in home health
