Patient Navigators are committed to removing the client's barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health. The Patient Navigator ensures that the patient is connected to prevention and treatment services, and also ensures that the patient adheres to the established plan of care.

Roles & Responsibilities:

Carries out tasks to execute the medical and support service plans including guiding patients to appointments for GI clinic and for their Procedure.

Ensures that medication adherence issues are addressed when giving prep instructions

Provides education and coaching for GI procedures.

Acts as the primary liaison with medical providers to ensure patient adherence. .

Coordinates logistics for plan adherence such as appointment reminders and arranging transportation.

Documents all patient encounters and enters all data into the EMR

Performs other tasks as required.

QUALIFICATIONS:

  1. Education Required - HS Diploma or Equivalence
  2. License/Certification/Registration(s) Required - Graduate from an Approved Medical Assistant Program
  3. Experience/Skills Required
  • Previous experience in a medical setting or 2 years relevant work experience preferred
  • Excellent communication/ interpersonal skills
  • Bilingual preferred
  • Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.
