About the Facility

Join our in-house rehab team under supportive, experienced leadership
This Lake Wales facility provides skilled nursing, long-term care, and short-term rehab services in a patient-centered environment designed to help residents achieve their highest functional level.

⚙ Responsibilities

  • Conduct thorough functional assessments and develop individualized treatment plans.
  • Deliver high-quality, evidence-based therapy sessions.
  • Document treatment according to company and regulatory standards.
  • Collaborate with interdisciplinary staff to support resident goals and progress.
  • Educate residents and caregivers on exercises, mobility, and safety strategies.
  • Maintain safe treatment environments and adhere to all compliance, quality, and safety guidelines.
  • Participate in interdisciplinary meetings and continuous improvement efforts.
Qualifications
  • Bachelor's or Master's degree in Physical Therapy
  • Active Florida PT license
  • BLS/CPR certification (or obtained during orientation)
  • Strong communication, teamwork, and time-management skills
  • SNF experience preferred, but not required
