Occupational Therapist (OT) - Home Health - $2,500 Sign-On Bonus

Location: Mission Valley & Greater San Diego, CA
Service Area: San Diego County (In-Home Patient Visits)
Work Type: Full-Time
Compensation: $40-$63/hour, depending on experience

A well-respected home health organization invites passionate and driven Occupational Therapists to join our compassionate clinical team serving the Mission Valley and greater San Diego area. As a vital member of our home health team, you will evaluate patients' functional limitations, develop tailored treatment plans, and empower patients and caregivers with essential rehabilitation techniques. You will collaborate closely with a dedicated interdisciplinary team committed to delivering high-quality, patient-centered care.

Key Responsibilities:

  • Assess patients' functional abilities, limitations, and home safety needs through comprehensive evaluations.

  • Create and implement individualized occupational therapy treatment plans.

  • Educate patients and caregivers on therapeutic techniques, adaptive strategies, and equipment.

  • Document care accurately and promptly, adhering to industry standards.

  • Collaborate with nursing, physical therapy, and other disciplines to ensure seamless, effective patient care.

  • Travel within the designated area for in-home visits.

Required Skills:

  • Graduation from an accredited Occupational Therapy program.

  • Active state licensure as an Occupational Therapist in California.

  • Minimum of 1 year of experience as an Occupational Therapist.

  • Current CPR certification.

  • Valid driver's license, reliable transportation, and automobile insurance.

Additional Skills to Consider:

  • Experience with EMR systems.

  • Prior experience in home health.

  • Familiarity with Medicare home health guidelines.

  • Able to effectively prioritize and manage a home-based therapy caseload.

  • Skillful in developing functional, goal-oriented patient care plans.
