Job Summary

The Clinical Care Manager (CCM) plays a critical role by utilizing the nursing process to deliver individualized nursing care within our facility. You will collaborate with a diverse team of healthcare professionals to create, implement, and evaluate personalized care plans. Responsibilities include:

  • Documenting resident history, symptoms, medications, and care provided in compliance with established standards/regulations.
  • Employing technology to enhance resident care and foster collaboration among team members.
  • Delegating tasks and supervising unlicensed personnel to ensure high-quality care.
  • Providing leadership on departmental and organizational task forces and committees.
  • Demonstrating strong clinical reasoning and critical thinking skills to respond to varied healthcare needs.

As a Clinical Care Manager, you will function within the standards of nursing practice and ethics set forth by the American Nurses Association, ensuring a supportive environment for both residents and staff.

Qualifications

  • Bachelor's degree in Nursing strongly preferred.
  • Graduate from an accredited nursing program.
  • A minimum of two years of clinical experience preferred.
  • Current unencumbered RN license valid in the state of practice, with multistate licensure if applicable.
  • Basic Life Support (BLS) certification required.
  • Experience with the Resident Assessment Instrument (RAI) process preferred, but not required.
