A Joint Venture Between Select Medical and SSM Health
Position: Case Manager
Location: Bridgeton, MO
Schedule: PRN 4-6 hours per weekend and occasional weekday (Mondays or mid-week coverage) Works one day every weekend 4-6 hours doing weekend intake assessments and some weekday availability to jump in and help with case loads
Orientation: Needs to be available for 3-4 week orientation, 3-5 days a week for training/orientation if possible
Compensation: $34/PRN Rate

Responsibilities
The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Qualifications
Minimum Qualifications
- Current Licensure in Social Work or in a clinical or related discipline (RN preferred), per state guidelines for case management and/or discharge planning, required.

Previous experience in Case Management and Discharge Planning preferred.

CCM Certification Preferred.
