Description
San Mateo County Health is seeking an experienced and qualified Public Health Nurse (PHN) to serve in the California Children's Services (CCS) in the Family Health Services division. CCS is a tax supported statewide program that helps children and young adults up to 21 years old who have certain medical conditions. CCS provides services such as arranging, directing, and in some situations, paying for medical care, occupational and physical therapy, equipment, and rehabilitation.

Under the supervision of the Senior PHN, the PHN will act as the Public Health Nurse Case Manager to perform case management tasks that will assist the CCS client in the receipt of appropriate and timely access to community and specialized health care services. The PHN's duties will include the processing of authorization requests; care coordination of health care services with community health care providers; facilitation of physician and patient collaboration in supporting medically necessary treatment services via management education, support and advocacy. In addition, the Public Health Nurse will work with the Medical Therapy Unit (MTU) Team on mutual cases to enhance the medical and community coordination of care for the child.

The current vacancy is a full-time, limited-term position. The work location is 801 Gateway Blvd., South San Francisco, CA 94080. Duration of the assignment is expected to be one year.

The ideal candidate will have strong nursing assessment skills, interpersonal skills, and analytical/problem solving skills.

All candidates must have current CPR or BLS certificate.
Responsibilities include:
  • Assess children's health status through home visits, history taking, observation, and developmental screening tests using the nursing process.
  • Monitor and ensure children's health, dental, and developmental follow-up needs are met.
  • Assist social workers in managing children's health and medical needs through working with foster parents, legal guardians, probation officers, and group homes.
  • Counsel, teach, and coordinate health and social services provided to client/family.
  • Participate in defining and addressing community health needs through consultation, recruitment of providers, and development of resources.
  • Participate in developing instructional materials.
  • Keep statistical records and write reports.
  • Inform clients regarding medical needs and Medi-Cal.
  • Link clients to medical, dental, and other community resources.
  • Present training to and collaborate with physician offices.
  • Participate on a multidisciplinary team.
  • Perform related duties as assigned.
NOTE: This is a limited term, at-will position. Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan. The eligible list generated from this recruitment may be used to fill future extra-help, term, unclassified, and regular classified vacancies.
Qualifications

Licensure/Certification:
  • California license as a Registered Nurse.
  • California Certificate as a Public Health Nurse.
Education and Experience:
  • Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.
Knowledge of:
  • Principles and practices of public health nursing, with emphasis on field and clinic activities.
  • Problem solving through nursing process.
  • Technical nursing procedures.
  • Health and social services resources.
  • Collaborative health planning with multi-disciplinary teams.
