Schedule: PRN / As Needed
Team Environment: Supportive, collaborative, and resident-focused
Arbor Post Acute is seeking a skilled and compassionate PRN Physical Therapist to join our interdisciplinary rehabilitation team. This role offers flexibility while allowing you to make a meaningful impact by helping patients regain mobility, independence, and quality of life.
What You'll Do
- Evaluate patients promptly following physician referral and develop individualized treatment plans
- Deliver high-quality physical therapy services in accordance with physician orders and best practices
- Document evaluations, daily treatments, progress notes, and discharge summaries in compliance with state and payer requirements
- Collaborate with physicians, nursing, families, and the interdisciplinary care team to support patient progress
- Supervise Physical Therapy Assistants, aides, and students in accordance with state practice acts
- Participate in care conferences, utilization review meetings, and rehabilitation team discussions as needed
- Provide education and training to patients, families, and facility staff to support continuity of care
- Assist with discharge planning and caregiver training
- Recommend and facilitate ordering of durable medical equipment to promote patient independence
- Support staff development through in-services and ongoing education
- Bachelor's degree in Physical Therapy (Master's or Doctorate preferred)
- Current, active Physical Therapist license in good standing
- Experience in a post-acute or skilled nursing setting preferred
- Proficiency with electronic documentation systems such as Casamba and PointClickCare
- Strong communication, organizational, and teamwork skills
- Ability to build positive relationships with patients, families, and colleagues
