Role Responsibilities
- Deliver extensive, ongoing healthcare services to individuals and families, encompassing all ages from children to seniors.
- Address chronic diseases, promote preventive health strategies, manage acute illnesses, and provide support for mental health issues with a patient-first mindset.
- Perform thorough physical assessments, assess diagnostic imaging, and formulate customized treatment plans.
- Work collaboratively with multidisciplinary teams, including care coordinators, mental health specialists, and various healthcare providers to ensure comprehensive patient care.
- Engage in quality enhancement initiatives aimed at improving patient outcomes and overall healthcare performance.
- Utilize electronic medical records (EMR) systems for precise, updated patient information management.
- Deliver care in both in-person and virtual formats to meet patient preferences and needs.
- MD or DO from a recognized medical school.
- Successful completion of an accredited Family Medicine residency program.
- Board Certification in Family Medicine.
- Possession of (or eligibility for) an active medical license in Texas.
- BLS and ACLS certifications are preferred.
- Exceptional diagnostic and clinical skills with a strong focus on preventive care.
- Familiarity with value-based care delivery and a desire to pursue continuous improvement.
- Strong communication, teamwork, and interpersonal abilities.
- Comfortable with technology, especially EMRs and telehealth services.
