As a Nurse Practitioner specializing in hospice and palliative care, you will work closely with seriously ill patients to manage their symptoms and ensure that their treatment aligns with their personal goals and values. Your collaboration with a supportive team of clinicians will be vital in delivering exemplary care, regardless of where patients call home.

What We're Looking For:

  • A dedication to serving patients and families with compassion
  • Education: Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
  • Experience: 2 years of experience within the last 5 years in chronic disease management or long-term care; hospice background preferred
  • Requirements: Reliable transportation, ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs
