RN Case Manager Hours Monday to Friday 8am - 5pm County Covered: Washington County/Chisago County/Anoka County RN Case Manager Compensation The pay for this position is $50.00-55.00 an hour paid weekly Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates RN Case Manager Responsibilities Perform comprehensive assessments of high-risk members Collaborate with primary care providers to implement individualized care plans Continuously evaluate appropriate, cost-effective levels of care Coordinate care transitions with physicians, social workers, discharge planners, and claims teams Arrange non-medical support services (e.g., housing, transportation) to support treatment adherence Engage specialty resources as needed to improve patient outcomes Maintain detailed documentation of clinical, functional, and fiscal outcomes Identify opportunities for health promotion and illness prevention Act as a patient advocate while ensuring privacy and confidentiality Intervene promptly to prevent or minimize adverse patient outcomes RN Case Manager Requirements Licensed RN Home Health/Hospice experience or case management highly preferred If you think RN Case Manager Position is a good fit for you, please reach out to me - feel free to call, e-mail, or apply to this posting
