Schedule: Monday-Friday, 9:00 AM-5:00 PM

Pay:

• Local: $47/hour

• Mileage: $0.70/mile reimbursement

Duration: 13 weeks to start (Temp-to-Perm & Direct Hire options available)

About the Role

Amergis Staffing is proud to partner with a highly respected Adult Day Health organization dedicated to helping elderly and disabled individuals live safely and comfortably at home. This program provides personalized care plans, strong clinical oversight, and ongoing case management to enhance the lives of participants and caregivers.

As an Clinical Admissions LPN , you’ll play a key role in assessing, planning, implementing, and evaluating care for program participants. You’ll maintain accurate records, complete assessments, implement provider orders, and collaborate with an interdisciplinary team to ensure quality outcomes.

Key Responsibilities

• Conduct monthly in-home visits, providing caregiver education and skills training.

• Perform semi-annual visits: update care plans, review goals, reconcile medications, and audit caregiver tools.

• Address concerns related to home environment, psychosocial status, and physical health changes.

• Complete all required documentation and communicate professionally with PCPs and providers.

• Participate in multidisciplinary team meetings and collaborate across disciplines.

• Demonstrate compassion and cultural humility toward diverse communities.

• Mentor and support team members to foster a collaborative environment.

Qualifications

• Active Massachusetts LPN license in good standing.

• 2+ years of nursing experience preferred (new grads welcome).

• Experience in clinical assessments required.

• Strong clinical judgment and care delivery skills.

• Proficiency with Microsoft Office Suite; comfort with telehealth tools.

Preferred:

• EMR experience.

• Care management background.

• Work occurs in residential and community settings.

• Travel required; valid driver’s license needed.

• Member-facing role with direct in-person care.

• Contract requirements: TB test and physical.
