Case Manager

Location: On-Site, Phoenix AZ
Hours: M - F 8a - 5p
Pay: $20/hr - 23/hr Depending on years of experience (Set tiers)
Job Description:
We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will have 2+ years of experience in behavioral health and a passion for supporting individuals on their recovery journey.
Responsibilities:
- Provide individualized case management services to clients.
- Develop and implement recovery plans in collaboration with clients and their support systems.
- Conduct assessments and facilitate access to resources and services.
- Monitor progress and adjust plans as needed to ensure successful outcomes.
- Bachelor's degree in social work, psychology, or a related field preferred.
- Minimum of 2 years' experience in behavioral health or a related field.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Must have a valid driver license and reliable transportation
- Must be willing to travel in their own vehicle (mileage reimbursed)
