We are seeking a dedicated Registered Nurse (RN) or Physician Assistant (PA) to join our Population Health Division. The ideal candidate will have experience or strong interest in value-based medicine, high-risk patient management, and addressing gaps in care. This role plays a key part in improving health outcomes for marginalized and underserved populations, supporting our mission to deliver equitable, high-quality care.

Key Responsibilities

  • Monitor, track, and close gaps in care across value-based programs.
  • Conduct outreach and follow-up for high-risk patients to support care compliance.
  • Review medical records and clinical data to identify required preventive screenings, chronic disease management needs, and overdue health measures.
  • Collaborate with providers, care coordinators, and the quality team to improve population health metrics and clinical performance outcomes.
  • Support patient education on chronic disease, preventive care, and care plan adherence.
  • Assist with the development and implementation of value-based care initiatives, including workflows that improve quality scores and patient engagement.
  • Participate in quality audits, documentation reviews, and reporting for payor-based value programs.
  • Promote health equity by helping remove barriers to care for underserved patient groups.

Qualifications

  • Active RN or PA license in New Jersey (required).
  • Experience with value-based medicine, quality measures, or population health strongly preferred.
  • Knowledge of HEDIS measures, preventive screenings, chronic disease management, or care coordination is a plus.
  • Strong communication, assessment, and patient engagement skills.
  • Ability to work independently, manage follow-ups, and prioritize patient needs.
  • A passion for improving health outcomes in marginalized communities.
