We offer an array of medical and chronic care services and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

Homebase Medical is expanding rapidly into theSan Gabriel Valley, CA area. We are currently searching for an experienced full-timeAdvanced Practice Clinician (Physician Assistant or Nurse Practitioner)to join our expanding house call practice. You will be responsible for primary care in the home setting, Board and Care, and Assisted Living Facilities.

**This position covers patients in and around West Covina, Azusa, Glendora, Arcadia, CA areas in the San Gabriel Valley** This is a full-time 40 hours per week opportunity.

Key Duties:

• ConductAnnual Wellness Visitsfocused on patient education

• DeliverChronic Care Managementto improve long-term outcomes

• Perform comprehensive in-home visits including:

• Medication review & chronic illness management

• Medical history updates & physical exams

• Labs, diagnostic screenings, and post-hospital follow-ups

• Primary care management & palliative care services

Qualifications:

• ActiveNurse PractitionerorPhysician Assistantlicense in the State of California

• Board certification (Nurse Practitioner or Physician Assistant)

• DEA licensure

• Prior experience preferred (but passion and commitment are just as important)
