Your Responsibilities:

  • Deliver comprehensive care to senior patients in line with clinical standards.

  • Collaborate with the care team to ensure excellent coordination of services.

  • Oversee referrals and manage coordination with hospitals and health services.

  • Engage in daily team huddles to discuss patient care.

  • Accurately document patient care with support from coding specialists.

  • Participate in on-call rotations and contribute to center growth initiatives.

  • Ensure all services comply with regulatory and accreditation standards.

  • Focus entirely on patient care, including face-to-face time and necessary administrative tasks associated with that care.

Our compensation package includes a base salary with potential earnings exceeding $300K and an attractive sign-on bonus of up to $60K.

We are hiring for the CenterWell Eastside Clinic:601 Versailles Rd, Frankfort, KY 40601

Qualifications:

  • Medical degree (MD or DO) from an accredited institution.

  • Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.

  • Active medical license or willingness to obtain one in the state of practice.

  • Excellent communication skills, both written and verbal.

  • Strong interpersonal abilities and a commitment to teamwork.

  • Enthusiasm for learning and adapting to a value-based care model.

  • Basic computer literacy, including familiarity with email and electronic medical records.

  • Participation in tuberculosis screening program is required.

Preferred Qualifications:

  • Active DEA and Medicare/Medicaid provider numbers.

  • Experience in managing Medicare Advantage patient panels.

  • Knowledge of Medicare guidelines and quality indicators.
