Position will cover Brooklyn and Queens regions.
MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.
- Assesses patient psycho-social issues to ensure adequate delivery of home care services in order to meet agency goals and required objectives
- Obtains psycho-social history and conducts evaluation and assessment of patient including adequacy of patient support system
- Provides supportive counseling in regards to emotional, psychological and concrete needs including discharge planning
- Interprets and provides information about available benefits, entitlements and community programs to patients and family
- Counsels patients and receommends community based resources regarding their psycho-social needs
- Master's degree in Social Work required
- New York State Social Work License required (LMSW or LCSW)
- Knowledge of the operations of community-based health and social services programs, concrete services and government entitlements required
- Home Care experience preferred
- Driver license and car required
