Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
• Provide skilled nursing care under the direct supervision of the clinical supervisor
• Complete evaluation tasks including vital signs and medication reviews
• Coordinate with physicians, therapists, and other individuals in the client's care plan
• Assess and chart observations of the patient's condition at each visit
• Maintain complete and accurate documentation of nursing care
• Work collaboratively with all members of the health care team
• Provide education to patients and families on proper home health care procedures and strategies
• Record patient's medical information and vital signs
• Monitor, record, and report symptoms or changes in patient's conditions
• Consult and coordinate with health care team members to assess, plan, implement, or evaluate patient care plans
• Administer local, inhalation, intravenous, or other anesthetics
• Prepare patients for and assist with examinations or treatments
• Modify patient treatment plans as indicated by patient's responses and conditions
• Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition
• Monitor all aspects of patient care, including diet and physical activity
• Assess the needs of individuals, families, or communities, including the assessment of the patient's home or work environments, to identify potential health or safety problems
• Engage in research activities related to nursing
• Perform other duties as assigned
Minimum Qualifications (Knowledge, Skills, and Abilities)
• Associates degree required; Bachelor's degree from school of nursing preferred
• Current and valid Licensed Practical Nurse license
• One year of LPN experience
• Current CPR certification required
• Valid driver's license required
• Current automobile registration and insurance required
• Strong interpersonal skills that build personal rapport and establish trust with the patient
• Computer literate
• Strong organizational skills
• Excellent written and communication skills
• Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
