What you will do:

The RN Case Manager team Lead, in collaboration with the clinical team, provides discharge planning coordination and intervention. The RN Case Manager Team Lead will perform concurrent and retrospective review of medical records for purposes of utilization management and compliance with requirements of external review agencies including governmental and non-governmental players. The RN Case Manager Team Lead strives to promote patient wellness, improved patient outcomes, efficient utilization of health services, and minimize denials of payment. The RN Case Manager Team Lead is responsible for supporting the Director of Care Coordination in guiding and managing the Case Management Team by providing direction, support, motivation, ensuring effective communication and collaboration within the team.

Required Education:

Graduation from an accredited School of Nursing

Preferred Education:

Bachelor of Science in nursing

Required Skills:

Basic computer and keyboarding

Preferred Skills:

Meditech Electronic Medical Record.

Required Experience:
Three years acute care nursing experience

Preferred Experience:
Case Management experience
