Integrated Case Manager for Population Health

Location

This is a 13-week contract position.

Schedule

Days, 8 AM - 4:30 PM, rotating weekends.

Job Description

The Integrated Case Manager for Population Health is a vital member of the patient-centered care team, responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation, and advocacy for comprehensive health care needs. This role focuses on optimizing health outcomes for patients who require complex management strategies after critical events or diagnoses.

Responsibilities

  • Conduct comprehensive assessments of patients and their families to evaluate biomedical, psychological, social, and functional needs.
  • Develop personalized care plans to enhance the patient care experience.
  • Engage patients and families through advocacy, ongoing communication, and health education.
  • Utilize judgment and motivational interviewing techniques to help patients overcome challenges.
  • Provide counseling on treatment decisions and end-of-life issues, including Advanced Care Planning.
  • Coordinate transitions between care settings to ensure a seamless experience.
  • Collaborate with care team members to support patients with complex medical and social needs.
  • Evaluate and modify care treatment plans as necessary.
  • Participate in process improvement activities to enhance patient safety and care quality.
  • Facilitate interventions for cases involving abuse or neglect.
Required Experience / Certifications / Licensure
  • Minimum of 5 years of experience in acute inpatient case management and discharge planning.
  • Strong computer skills, including experience with Epic EMR.
  • Nursing degree or MSW.
  • BLS certification through AHA.
  • Current valid Michigan RN or LMSW licensure.
  • Recent acute inpatient case management experience within the last two years.
