Per Diem Social Worker needed for Boston Children’s Hospital, Longwood Primary Care Clinics
Key Responsibilities:
-
Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.
-
Performing psychosocial and risk assessments, assigning diagnoses, and developing and implementing interventions and treatment plans.
-
Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan
-
Implementing and following up on treatment and provision of needed services.
Minimum Qualifications
Education:
- Master’s degree in social work
Experience:
-
Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.
-
Experience with social services agencies, understanding of social determinants of health and trauma ideal.
-
Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus
Licensure/Certifications:
- Active LICSW in the state of Massachusetts
Office/Site Location:BostonRegular, Temporary, Per Diem:Per Diem Remote Eligibility :Part Remote/Hybrid Pay Range:$26.62-$42.59 Hourly
