LPN Pediatric Home Care Nurse

As a Licensed Practical Nurse providing Home and Community Based Services, you will deliver essential nursing care as prescribed by physicians to acutely and chronically ill patients in their homes. Your responsibilities will include meticulous recording of clinical notes and coordinating patient care activities to ensure the highest quality treatment in your designated service area. You will work under the supervision of a Registered Nurse or designated supervisor.

Primary Responsibilities:

• Deliver high-quality clinical services within your scope of practice, adhering to state nursing laws and infection control procedures, while following the patient care plan and collaborating with the healthcare team.

• Thoroughly document patient visits in the electronic medical record, ensuring compliance with regulatory and payer requirements, using the agency's electronic visit verification program as applicable.

• Engage actively in the agency's care coordination process by reporting patient needs and changes in condition promptly, attending patient case conferences, and effectively communicating with patients, families, physicians, clinical colleagues, management, and community case managers as relevant.

Required Qualifications:

• Valid and unrestricted Licensed Practical Nurse licensure in your state of practice.
• Current CPR certification.
• A valid driver's license and vehicle insurance, access to a dependable vehicle, or reliable public transportation.
• Ability to provide care in diverse home environments regardless of clients' age, race, creed, color, sex, disability, or financial condition.
• In Louisiana:
  • Current LPN licensure without restrictions.
  • At least 1 year of experience as a Licensed Practical Nurse.

Preferred Qualifications:

• Experience in home care.
• Flexibility to work various hours to meet patient needs.
• Strong ability to manage multiple tasks simultaneously.
• Proficiency in working independently.
• Excellent communication, writing, and organizational skills.

The hourly pay for this position ranges from $20.00 to $35.72 for full-time employment, depending on factors such as local labor markets, education, work experience, and certifications.
